The Management of Severe Adult Scoliosis with Posterior Spinal Fusion


Abstract
It is felt that the management of severe adult scoliosis with posterior spinal fusion should be different from congenital or idiopathic scoliosis in childhood. But there is few report concerning the management for severe adult scoliosis with posterior spinal fusion, even though there. are a few reports dealing with rigid or congenital scoliosis (e. g. congenital unilateral bar)
We recently experienced a case of 126¨¬¡Æ curve from the 11th dorsal spine to sacrum with posterior spinal fusion from 10th dorsal spine to 5th lumbar spine, which is presumed to be an acquired form. The patient is 29 years old male whose parents noted their son to have a mild curvature in the spine at his age of 3, and then increased progressively to this remarkable curvature. The patient was admitted with the chief complaint of low back pain and easy fatigability, especially in walking at the Department of Orthopedic Surgery, Seoul National University Hospital on Aug-26-¢¥ 75.
The 126¨¬¡Æ initial curve was corrected to 82¨¬¡Æ after three stage procedures that is anterior discectomy from Ll to L5, posterior osteotomy at the level of Ll, and L2 following by halo-femoral traction and then posterior spinal fusion with Harrington¢¥s instrumentation.
